Quinnipiac Athletic Communications New Student-Athlete Bio Form
The Family Educational Rights and Privacy Act of 1974 requires that Quinnipiac University receives your consent to disseminate information contained in your student file. Information obtained from your records will assist with compilation of personal information for use in athletic department publications (brochures, programs, press releases, etc.). Your consent also gives the office permission to nominate you for honors pertaining to academic and athletic success. Also, the Buckley Amendment requires us to ask that you agree to this release form. By submitting this form you authorize the Quinnipiac University Athletic Communications office consent to review and disseminate any information in your educational/athletic records to third parties for general public relations purposes. Additionally, you further give Quinnipiac University Athletic Communications permission to use my likeness from photographs and videos taken at official Athletic Department events. The information will be used in official department publications, social media sites and websites, for purposes of nominating for honors programs, and for general media relations purposes only.
